Health West is happy to partner with the Great Rift Rural Health Network on the “My Family, Mi Familia” project! Taking care of your health is the first step in taking care of your family. If you or a member of your family has a history of heart disease and diabetes, we want you to join us and learn about a new program that can improve both your health and your quality of life.
This is a 3-year program that will track your health data while providing many free health education opportunities throughout each year. We will host health events and free health screenings throughout the year thanks to our mobile blood pressure unit.
We are looking to recruit adults with a family or personal history of heart disease and/or diabetes to participate in our free health education program. Participants should live in Power County or Aberdeen, Idaho.
All you have to do is wear the complimentary Garmin Vivofit 4 and attend occasional check-ins and health education classes, such as food and nutrition classes. You’ll receive prizes and rewards for your participation and for each class you attend.
We will help you find a primary care physician and access healthcare services.
